I understand and am aware that risks may arise from my child’s participation in Expressions Film Studio acting classes. I hereby assert that my child’s participation is voluntary and that I knowingly assume all such risk. I also understand that my child will not be supervised outside of class time and it is my responsibility to arrange for pick up, transportation and supervision when class is done. By signing this waiver I release Expressions Film Studio from any claims of injury for my child resulting from participation in any class, program or activity at the studio.

I grant permission to Expressions Film Studio to film my child for the sole purpose of training for film acting. I also grant permission for my child to be photographed and to have these photographs used for Expressions Film Studio promotional activities. This may include a future website, brochures and videos.
